Lifespan, Inc. Lifespan offers In-Home Care for seniors and disabled adults, Care Management by RN's and Social Workers, and Life Enhancement support by uniquely skilled Activity Coordinators. Our goal is to maximize the health, safety, comfort, and independence of our clients. Lifespan has delivered compassionate client-centered care to our local community since 1983. Lifespan caregivers and care managers follow all recommended COVID-19 precautions to ensure the safety of our clients, their families, and themselves. HOME CARE: Lifespan's home care aides provide typical daily care happenings such as bathing, dressing, hygiene care, meal preparation, as well as shopping, errands and dementia support. We work closely with local hospice organizations when such care is needed. All Lifespan home care aides are trained, bonded, insured, undergo an extensive background check and are supervised by our professional care managers. CARE MANAGEMENT: Our highly experienced and professionally trained care managers monitor health conditions, coordinate with health care providers, perform safety assessments, manage medications, provide support to family caregivers, and supervise our personal care aides. Lifespan can also assist with planning for transition to home from hospitals and nursing homes. Working families and family members living out of town report great comfort in knowing that our professional care managers are keeping tabs on their loved ones and helping to prevent crises. SOCIAL SUPPORT: Our newest program, Well-Being, offers life enhancement happenings to bring joy back into the lives of seniors who are less able to do the things that that have brought them pleasure in the past. Specially trained activity coordinators deliver individualized client-centered happenings such as socialization, recreation, home organization, and more, including medical appointment escorts.
