Assured Horizons FV 4 is a family-style residential care facility dedicated to providing superior care for seniors with Alzheimer's, stroke, and other medical conditions. Located in Fountain Valley, the facility features six intimate homes that boast security cameras, bright parabolic lighting, bidet toilets to prevent UTIs and infections, living air machines, locked garages, and fire safety training. In comparison to other care facilities that use large, 'home-like' environments, Assured Horizons provides a more personalized experience with a maximum of six residents and two daily caregivers. This allows for individualized attention and opportunities to meet residents' needs, while providing families greater peace of mind. Additionally, Assured Horizons upholds a caring approach that is rooted in kindness, love, and professionalism.

Assured Horizons FV 4 Assured Horizons‘ caring approach starts from the heart. We are dedicated to serve and care for each resident as a family member. That means kind, loving and professional care in a warm and comfortable home. Every detail of our care, the staff, our homes, the food, medication supervision, happenings and more is based on this caring approach. Most professionals agree that memory impaired individuals and medically frail individuals function better when they live in smaller surroundings with a familiar group of people. As a result, we refuse to have any ‘home-like’ facilities. We have real homes, just like the ones our residents have lived in during their entire lives. Our beautiful homes are airy and have a maximum of 6 members and at least two daily caregivers. All care is coordinated and supervised by our Nurse Administrators. This allows community members to remain in our caring homes and avoid institutional settings altogether. We provide superior care to assure our community members the highest quality of life and to provide their families peace of mind. What Makes Us Special Assured Horizons maintains high staffing ratios which are essential in the proper care of individuals with Alzheimer’s Disease and related disorders. These high staffing ratios promote individualized attention and opportunities to address residents’ individual needs and personal desires. This is accomplished in each home by having no more than 6 residents and at least 2 caregivers. These staffing ratios also increase the long-term stability of Assured Horizons’ caregivers assisting residents.
