Assured Horizons FV 5 is located in Colville Street Fountain Valley CA and is dedicated to providing superior care to seniors suffering from Alzheimer’s Disease, stroke, and other difficult medical conditions. All homes have a maximum of 6 residents and at least two daily caregivers, with all care coordinated and supervised by our Nurse Administrators. Our staff is committed to developing the highest professional standards in residential care for the elderly. All homes have security systems, camera monitoring, bright parabolic lighting, are close to a park, have bidet toilets to prevent U.T.I’s and infections, living air machines, locked garages, external door exits from rooms, fire safety training, personal/fall safety alarms. What makes us special is maintaining high staffing ratios in each home, with no more than 6 residents and at least 2

Assured Horizons FV 5 Assured Horizons‘ caring approach starts from the heart. We are dedicated to serve and care for each resident as a family member. That means kind, loving and professional care in a warm and comfortable home. Every detail of our care, the staff, our homes, the food, medication supervision, happenings and more is based on this caring approach. Most professionals agree that memory impaired individuals and medically frail individuals function better when they live in smaller surroundings with a familiar group of people. As a result, we refuse to have any ‘home-like’ facilities. We have real homes, just like the ones our members have lived in during their entire lives. Our beautiful homes are roomy and have a maximum of 6 community members and at least two daily caregivers. All care is coordinated and supervised by our Nurse Administrators. This allows members to remain in our caring homes and avoid institutional settings altogether. We provide superior care to assure our residents the highest quality of life and to provide their families peace of mind.
