An In-Home Visit with You, Your Loved One and Family Members We offer families the opportunity to get to know us better during a no-cost, no-obligation, in-home visit with one of our staff members. This visit can take place in a private residence, skilled-care facility, and hospital or rehabilitation center.

During this visit, we will further evaluate the needs of your loved one and your family, set goals for care, perform fall prevention and home safety checks and discuss costs and available funding options. In-Home Care Assessment Once all parties agree on an appropriate level of care and to begin home care services, one of our experienced Care Managers will visit to work with the client and/or family to develop a Care Plan to met the client needs. This is a personalized care plan that will be followed by the caregiver at every visit.
