  While visiting I saw dirty floors that looked like urine that had dried on them, the bathrooms were totally out of toilet paper and had a terrible smell in them, and I never saw a nurse stop to wash her hands at any time.

  I have friends that come in here and not one says it is anything but clean: have a new manager 4 star: food 1 star, it is bad, but one meal a day would be something you could eat.
