Our trained, friendly staff is always available to lend a hand when needed.We develop a Personalized Care Plan in which needs are carefully assessed to determine the care options that best suit each individual. There is no “one size fits all” approach to resident care. The combined elements of personalized assistance and the privacy of one’s own apartment promote independence and vitality. Elite Care at Palo Alto Commons- This special level of care adds support beyond what is typically given in assisted living. Elite Care has the highest staff-to-resident ratio in our community, and delivers one of the highest levels of assisted living care in the Bay Area. Residents have their own private garden apartment. Our members receive care, comfort and assistance with the happenings of daily living from our highly trained staff, as well as care based on their Personalized Care Plan. The Elite Care team is also equipped with standing lift devices for assistance with transfers and can provide full assistance with dining needs. Designed to cultivate as much independence as possible, Elite Care helps residents rediscover their interests and happenings, helping to re-integrate them into their lives today. It bridges the divide between assisted living and higher levels of care. The Meadow Wing at Palo Alto Commons is a specialized and dignified program for residents with Alzheimer’s disease or other forms of dementia, providing a full daily schedule of engaging and meaningful activities in a secure, friendly, homelike environment. Focused Care – A Palo Alto Commons Exclusive. Focused Care is a unique, flexible option for community members with a high level of care needs, but varying social and dining needs. The higher staff-to-resident ratio allows for more extensive care and assistance. Residents engage in social and dining programs in the Assisted Living, Meadow Wing or Focused Care programs, depending on their amount of functioning.
