This includes: Care coordination with all of the residents of the client’s healthcare team Scheduling medical and therapy appointments and arranging for transportation Medication Reminders Home Safety Evaluation Recommendations for and placement of assistive devices in the home 24 hour access to speak with an on Call Nurse if needed. Online Portal to view updates on client and caregiver activity Our goal is to keep our clients secure, happy and healthy in their homes
