Our registered nurse will do a complete diagnostic assessment based on the physician’s Plan of Treatment, as well as client and family needs in order to create an individual Plan of Care. The nurse will then instruct and supervise the assigned home health aide on the necessary care. Our nurses will provide re-assessments on a regular basis and maintain constant communication with the physician, family and agency.
