About Pond Home
Pond Home is a licensed residential care facility regulated by the Department of Public Health and operated by a Licensed Nursing Home Administrator. The facility includes a 12-bed Supportive Nursing Care section for residents with higher medical needs.
Residential care residents receive meals based on doctor-recommended diets, with meal planning done according to preference and reviewed by a registered dietitian. The Supportive Nursing Care section operates with 24-hour on-site registered and licensed nursing staff who assess medical conditions, manage medications under physician direction, monitor medication reactions, and communicate with residents' physicians as needed. Certified nursing assistants provide personal care and safety checks around the clock. Both sections include daily housekeeping and laundry in the care fee.
The Supportive Nursing Care section provides most skilled nursing services except feeding tube care and intravenous administration; residents requiring these services cannot be admitted. Rehabilitation services are offered at a less intensive level than skilled nursing facilities and may be covered under Medicare Part B. The facility accepts residents who are bed-bound or require staff assistance with feeding, transfers, bathing, and incontinence management. The facility also cares for residents with mild dementia living outside a secured unit.
